Lexa Records is a third-party professional disclosure management company which partners with various hospitals, clinics, and other medical practices to help accelerate and ease their medical records management.
If you are a patient being sent to us for further information on your medical records request please call 817-421-4700 or email info@lexarecords.com and someone should be able to help provide status on your individual medical records request.
We are open Monday through Friday, 8 AM – 4 PM Central Time

The invoice number can be found in the upper right corner on your invoice underneath the Lexa Records Logo.
The reference number can be found in 3 places: In the subject line of the email, in the body of the email, and at the bottom of your invoice above the words “Thank you”.

Yes, we accept all major credit card payments.
To make a credit card payment you can either use the link provided by QuickBooks to pay through QuickBooks or you may click on the Pay Invoice tab to go to https://securepayment.link/lexarecord/ to make your payment.
Yes, we accept checks.
Make your check out to Lexa Records.
Include your invoice number in the memo section of the check.

Yes, we accept EFT.
EFTs are offered to clients with permission from their project manager. Please contact your point of contact for more information.
