A statewide Arizona network with more than 70 physicians and oncology specialists required support migrating historical data and physician notes to an upgraded EMR system. With only one month remaining before go-live, Lexa Records partnered closely with network leadership and providers to design and approve specialty-specific templates, execute the migration, and ensure continuity of care for upcoming patient appointments following go-live.

Our Solutions

Physician and oncology notes in the legacy EMR were inconsistently formatted, requiring detailed analysis to identify variations across specialties and providers. Lexa Records assessed these differences, mapped content to the appropriate templates, and developed a structured process to ensure accurate data transfer into the new EMR. To meet the accelerated timeline, Lexa Records implemented a customized training and management model that enabled efficient, high-volume migration while maintaining accuracy and quality standards. All patient notes were successfully migrated to the new EMR at least one week prior to scheduled appointments.

Outcomes

The success of this initial pilot led to an expanded partnership, with Lexa Records also providing ongoing Release of Information (ROI) and Medicare Risk Adjustment Review (MRAR) support. Lexa Records: 

  • Migrated patient notes for 70+ providers across 28 locations 
  • Eliminated the need for the network to hire an estimated 15 full-time employees 
  • Supported approximately 350-450 patient appointments per day
